End of Year Notes From the Nurse
 If your child has medications at the school nurse office, these must be picked up by Fri. at noon. No medication can be left in the nurse's office over the summer, it is not temperature controlled and safety cannot be assured. Your physician must complete new Medication Authorization forms for next fall. These will be given to you when you pick up the medication.

If you have not completed and returned the immunization record for the required Tdap and Meningococcal vaccines for your child to enter 6th grade, please send the record to the Central School Nurse as the health records are being transferred there. Over the summer also send the record to the Central nurse.
